Hi, I've been taking Nilotinib for about 3 months. I take them at 8 & 8 but would like to alter to 4 & 4. Is it best to do a gradual change rather than go straight to the 4 & 4 time. Any advice will be most appreciated. All the very best to everyone. Joe.

Having moved international time zones I've had to do this. Depending on who you listen to you can only slip 1 hour at a time or you can slip as much as 4. In your shoes I would probably take my pill at 8am, 6pm then 4am and then at 4pm - tansition complete. coming forward is probably easier than going back I.e. moving from 4 to 8.

Hi Joe, Chris has given some sound advice on how you might approach the change in timings.
I contacted novartis for their advice and received the following replay from their EU medical lead:
"As far as I know there is no official guidance on this.
I think it is fine to make the change in one step – I would suggest starting with the 4pm dose rather than 4am as the data suggests a higher Cmax following the evening dose compared with the morning one – possibly due to decreased gut motility during sleep.
I would also emphasise adherence to the fasting window to minimise the possibility of any AE’s as this pt has only been on Nilotinib for a short time.

Hello Joe,
I recently asked my consultant at Hammersmith Hosp re changing my time from 3am and 3pm to 12 noon and 12 midnight. Advise was as follows:
Just change immediately , don’t worry about doing it gradually – it will minimise the duration of the change in your blood levels.
I made this change about a month ago and have had no issues with it. So I would say just do it asap- good luck .
