bosutinib

I'm about to start on bosutinib, having failed to respond to nilotinb.(some response but not good enough!) The leaflet says to take it in the morning with food, but I don't usually have a very large breakfast so I would rather take it in the evening after dinner. Does anybody have any experience and/or advice on the best time to take it? I' m well stocked up with anti diarrhea and anti sickness tablets but I really hope they will not be needed.

I started out taking it in the mornings but once i had established that diarrhea wasn't an issue (!) i changed and now take it after my evening meal. My consultant was fine with doing that and it suits me much better.

Thanks for your reply. I took the first one last night and I must say...the dreaded diarrhoea did not take long to hit! I'm not too bothered provided I'm not going to be sick as well- I just hate vomiting! I have the dubious honour of being the one and only patient in York to be on bosutinib so it's uncharted territory for both me and my consultant. How long have you been on it and is it working well for you?
Any other advice/information /personal experience much appreciated.

Oh no - that is really unlucky. Touch wood i really haven't suffered from that unless i have eaten spicy food as well. What dose are you on? I started in August on 300 but had to have a break as my liver decided to have a temper tantrum! I re started on 200 but with steroids to help the liver. Other than dry skin i have had no real issues so i am hoping the liver calms down. If the diarrhea gets too bad you could maybe drop to 100? There are people who seem to do ok on that dose

I'm on 500, full dose from the start - I had no major problems with nilotinib and I hope that my liver can cope with this one. First check up in a week. I've got the feeling this is going to be a bumpy ride,but provided I can carry on doing what I want/like to do (including a full time job that I really enjoy), I'm quite prepared to see it through.
By the way why did you switch to bosutinib? I hope it's working well for you

I think my consultant avoided the full dose as side effects can be such an issue. This is drug number five for me - have had issues on all of them so i am hoping this will be the one. Let me know how the weekend goes!

Thanks for asking. Things are fine,everything settled down very quickly and no further problems. Spent a very nice day with my daughter in Newcastle, went for a walk followed by a good pub lunch. So far so good but very early days. There is still very little data on bosutinib and, from what I read,quite a high rate of treatment interruption and discontinuation due to side effects....time will tell I suppose.

Hi Luisa,

just to clarify why the data shows a high rate of discontinuation on bosutinib. During the initial trials a lot of patients experienced diarrhoea but instead of this effect being 'managed' with other therapy in order to keep patients on treatment and see whether it was either a transitory effect or if it would be an ongoing problem, most were taken off the drug. However, investigators found it was indeed an effect that a. could be managed and b. was transitory.

Overall bosutinib seems to be shaping us as a popular choice with clinicians and is certainly holding its own against other TKIs.

I hadn't really considered going. Oxford is quite a long way from York. I can't go the night before so I would have to travel on the day, setting off very early. I'll think about it
