pcr testing

What are peoples current experiences of pcr testing?

How often are they being done, and are they being done on peripheral blood (from a vein), or from a bone marrow aspirate?

I know that there's a debate about which is the most relevant ie blood or bone marrow result, but was wondering how much variation there is in practice in the different centres around the uk / europe.

Also, does anyone have experience of having a qualititative pcr showing up a negative result but a quantitative pcr done within a few months showing a 3 log reduction and not zero?

Paul

Hi Paul,

well we don't know yet, my husband has been recently dx.
From the information Dr. give us, he is being tested on blood at 3rd month and in month 6 for the bone marrow aspirate.

regarding levels of pcr, I'm not an expert, but I would say that in this case the most important is to detect levels on bone marrow as it's the place to look for myelocitic cells (when the levels are too high they started to appear on blood). But I'm not sure

On peripheral blood mine are suppose to be done every 3 months, but one got lost and one failed, so not an even run. Also they actually fall every 4 months - for example I had a BMB beginning of March - results back in April went for my consult - booked PCR peripheral 3 months from April - July. So July's will go results in August next appointment in November. My BMBs are currently annual.

Hi Paul

It is a coincidence that you are enquiring about PCR testing since I also had some questions following my recent appointment at Llandough - I would be interested to know if Anne's experience is the same as mine since we are both being treated by the same collection of Haematologists albeit at different sites. [I wonder if this is the right plural collective noun for Haematologists?? :-). Very facetious, sorry, I digress....]

My consultant, who is I understand Clinical Director for Haematology in Cardiff, told me that my PCRs would be done from the marrow, not the blood, and he would be continuing to do PCRs from BMAs every 3 months until, hopefully, I reach a 3 log reduction. Currently at 1.9 log. He didnt mention what would happen after 3 log.

As for qualitative negative vs quantitative 3 log - I'm definitely no expert and have a lamentably poor understanding of the whole thing, but wouldn't the maths of this suggest that it could be something to do with the sensitivity of the testing?? Quantative 3 log may be more sensitive than the qualitative test used previously.....

I have also heard others far more experienced in PCR results than I am, talk about PCRs bouncing around at very low levels. I would have thought this also could include negative to 3 log and back again, but once again I don't know enough to make an informed comment really.

What information was Anne given about testing in Cardiff?

Regards, Janet

Hi Paul,

Using the same blood draw or bone marrow draw I have the qualitative PCR come back as negative and the quantitative PCR as a 3 log reduction

This is pretty common.

Hi, Janet,

May I suggest that when those blood doctors pool (pun intended) their resources, they become a Coagulation of Haematologists?
